How Personally Identifiable Information Provided By You May Be Used. Except as described in this Privacy Statement, Onegevity Health only collects your PII through the Website when you choose to provide such information, such as when you create your Onegevity Health account. To create your Onegevity Health account, we require basic information from you, including your full name, mailing address, and a valid e-mail account. You have the option to provide additional information, including your professional licensing information if you are a health-care practitioner. Onegevity Health may use your PII:
De-Identified Data. De-identified data is information that has been stripped of all “direct identifiers” – that is, all information that could be used to identify an individual from whom PII was originally derived. According to the federal Health Insurance Portability and Accountability Act (HIPAA), there are 18 direct identifiers that can be typically present in PII, such as full name, mailing address, telephone number, medical record number, biometric identifiers, photographic image, etc. Onegevity Health ensures that all of the direct identifiers that are present in an individual’s PII have been deleted before the individual’s de-identified data is aggregated with the de-identified data from other individuals. Onegevity can use de-identified data for commercial purposes, such as selling de-identified data to third parties, and for non-commercial purposes, such as conducting and publishing scientific research or statistical analysis.
Except as de-identified data, Onegevity Health will not sell, rent, license, or trade your PII to any third party for their own direct marketing use unless we receive your express consent to do so. Unless you give us express permission to do so, Onegevity Health will not use your PII other than as specified in this Privacy Statement.
Surveys. From time-to-time the Website may request information from you via surveys. Participation in these surveys is completely voluntary and you may choose whether or not to participate and therefore disclose the requested information. Information requested could include information about your organization’s practices or your opinions. Survey information will be used for purposes of monitoring or improving the content, use, and satisfaction of this Website.
Disclosures to Third Parties Assisting in Our Operations. Web Server Logs and IP Addresses. An Internet Protocol (“IP”) address is a number that automatically identifies the computer/device that you have used to access the Internet. Onegevity Health may use IP addresses to conduct website analyses and performance reviews and to administer the Website.
In addition to using cookies, the Website may employ "pixel tracking," which is a common process used in connection with advertisements and/or internal tracking. Pixel tracking involves the use of pixel tags, an invisible tag that is placed on various pages of a website that tracks a user's activity. Pixel tags are not visible to users and consist of a few lines of computer code. Pixel tracking measures the effectiveness of advertisements by compiling aggregate and specific usage statistics. Onegevity Health may access pixel tags to identify the activities and interests of users that better enables us to match products, services, and offers to the user's interests and needs. Some Internet browsers include the ability to transmit “Do Not Track” signals. Because uniform standards for “Do Not Track” signals have not yet been adopted, Onegevity Health does not process or respond to “Do Not Track” signals.
